In case relationship is mentioned as Niece/Cousin/Uncle/Aunt only in the space provided for in the certificate, the Village Officer should give an explanation of the relation in the certificate itself. Niece/Cousin/Uncle/Aunt should be explained with clear relationship.

Certificates/applications showing mere Niece/Cousin/Uncle/Aunt without establishing clear relationship link will be rejected.

To apply online the candidate should have:‐

1. A valid E‐mail ID
2. Recent passport size photograph with white background (to be uploaded – jpeg – not more than 100 kb)
3. Signature of the candidate (to be uploaded – jpeg – not more than 100 kb)
4. Signature of the Parent (to be uploaded – jpeg – not more than 100 kb)
5. SSLC, Plus Two Certificates (copy to be upload – pdf – not more than 200 kb)
6. Self-attested copy of Aadhar Card (copy to be uploaded – pdf – not more than 200 kb)
7. Application Fee Rs.1,000/‐. You can pay online or by RTGS, cheque or demand draft (DD). Make cheques or demand drafts payable to “Assisi College of Nursing” at Mukkoottuthara. (Receipt to be uploaded – jpeg – not more than 100 kb)
   • The Cheque/Demand Draft should reach in the office within seven days after the submission of online application (address: The Principal, Assisi College of Nursing, Propose P.O., Erumely, Kottayam Dist.–686509).
8. Relationship Certificate (specify the relationship) to be issued by the Revenue Authorities to prove the relationship between the candidate (with official seal) (copy to be uploaded – pdf – not more than 200 kb)
9. Copy of valid Passport (front page & back page) attested by the Embassy/Consulate (copy to be uploaded – pdf – not more than 200 kb)
10. Copy of Visa/Green Card/OCI Card (Overseas Citizenship of India) attested by the Embassy/Consulate. (copy to be uploaded – pdf – not more than 200 kb)
    • If the sponsor’s employment is not stated in the sponsor’s passport/Visa/Green Card/OCI card, sponsor’s Employment Certificate issued by the employer, attested by the Embassy/Consulate OR if self‐employed, a statement in this regard attested by the Embassy/Consulate shall be produced.  (copy to be uploaded – pdf – not more than 200 kb)
11. NRI Certificate (attested by the Embassy/Consulate)
12. Declaration of the Sponsor attested by the Embassy/Consulate. (copy to be uploaded – pdf – not more than 200 kb) 
    • In case the sponsor is already in India, the affidavit shall be notarized by any of the notary public in a stamp paper worth Rs.200/-i.e. tuition fee and special fee of the candidate for the entire course period will be borne by the Sponsor. (Annexure‐I). The proof of arrival and departure to be provided (copy of respective pages of passport).
    • In case the sponsor is in other countries, the sworn affidavit shall be attested by the Embassy/Consulate. (Annexure‐I)
